I note the line number in each respective square. In my gameplay forwards can play 1 shift (6 cards) and d-men 1.5 or 2 shifts (9 or 12).
When a FAC card is flipped, or prior to, just slash a timeslot (you may want to use the shift cards the first game or two until you get your timing down).
When the first penalty occurs I note a p1 in the time slot amd note the penalty in the remarks line to the right. I also notate p1 in the slot when the penalty ends.
In the notes section to the right I also record SOG's, scoring, injuries and other related notes.
TV timeouts (yellow shade) count as a 2 minute rest during that time and I can choose basically any line except the one that was on the ice and call them rested (remenber the rulebook it states that things are not actually literal but that certain actions occur to get to that point? I assume play was stopped somewhere and the timeout was taken).
I have never played head to head. But I think this would be a great coaching addition to matching lines (Home has advantage on which line to use).
It adds a little more time to gameplay, but I am cutting that down game by game. I just hated counting cards for the line shift cards and penalities. If it takes just as long to use this chart as to set-up, I would rather use the chart. It's a combo timer & scoresheet.
